El caso de Laura

Después de no haber agotado los elogios sobre la obra de Marie-Andrée Sechehaye así como sobre la producción cinematográfica de Nelo risi, no nos es posible abordar el comentario del caso de Laura en las mismas condiciones que el caso de Renée. En efecto, ocupar simultáneamente el lugar del autor y de la crítica, equivaldría a ser, como se dice en Francia, juez en causa propria, un ejercicio al que no pienso aventurarme.

If I have praised Marie-Andree Sechehaye's work and the cinematographic production of Nelo Risi very much now, I can't approach the comment of Laura's case in the same conditions as Renee's case. Indeed, playing the part of the author and of the critic as well, would result in being as we say in France, juge and parti, an exercise which I would not try to do.

En cambio, utilizar este espacio de presentación para tener la posibilidad de ampliar los elementos sobre el caso de laura, es, claro está, una tarea a la cual no quiero sustraerme, ya que la dirección y guía del tratamiento psicoterápico de Laura, engendraron en mi vida un espacio cuya amplitud no he logrado limitar aún hoy, quince años después de nuestra primera cita. El caso de Laura es casi la réplica exacta del caso de Renée. Esto quiere decir que, a cincuenta años de distancia, los actos terapeúticos lograron un resultado similar, como un desafío a este medio siglo de investigación psicoanalítica y como si la inspiración de la Sra. Sechehaye no hubiera perdido nada de su vigor inicial. Así, los factores puestos en juego alrededor del caso de Laura, se han hallado muy subordinados al contexto que se desarrolló antes alrededor de la historia de Renée, incluso si el lapso de tiempo que les separa pueda parecer largo. Con el recurso al acto como procedimiento terapeútico y con la demostración de su eficacia terapeútica, la Sra. Sechehaye se libró a un ejercicio que además de producir un verdadero efecto de seducción sobre el público, correspondía a un modo de hacer sumamente audaz, dado el clima psiquianalítico de la época.

On the other hand, using this space of presentation to have the possibility of developing about Laura's case is naturally a task to which I could not conceal myself. The conduct of Laura's psychotherapic treatment has had such an influence on me that fifteen years after our first meeting, I still can't appreciate the repercussions. The Laura's case is almost as identical as Renee's case. It means that in fifty years of interval, the therapeutic acts ended in a similar result, like a challenge to this half-century of psychoanalytical research, and as if Sechehaye's inspiration had not lost anything of first dash. Thus, the factors concerning Laura's case have depended on the context développed around Renee's story. Even if the time which separates them can appear long by using the act as therapeutic process and by the demonstration of her therapeutic effectiveness, Mrs. Sechehaye devoted herself to an exercise which, in addition to the fact of having produced a true effect of seduction on the public, corresponded, in the psychoanalytical climate of the time, with an extremely daring step.

A partir de entonces las reacciones suscitadas estuvieron a la altura de las circunstancias : fueron excesivas, dobles, incluso ambivalentes y bastante pasionales. La prensa se hizo eco de ellas suficientemente para que se pudieran distinguir dos corrientes. Hubo quienes se apropiaron de la historia de Renée por convicción y se identificaron a la Sra Sechehaye : Yo habría hecho lo mismo. Y hubo quienes, más reservados o más prudentes intentaron resistir a este efecto de seducción basándose en los textos de Freud, explicando que si un intento de cura consistía en paliar a destiempo antiguas carencias mantenidas en suspenso, era, a pesar de todo, un poco reductor para el psicoanálisis: el artículo de Henri Chapier en combat, el 24 de Octubre de 1969, es a este propósito un ejemplo sorprendente.

Consequently, the reactions were excessive, double even ambivalent and rather passionnate. There were so many articles in the press that we can identify two currents. Those who adapted the story of Renee by conviction and who identified to Mrs. Sechehaye : I would have done the same. And those who, more reserved or more careful tried to resist to this effect of seduction by clinging to the text of Freud and by explaining that if a company of care consisted in mitigating in afterwards old deficiencies remained in suffering, it was a little reducing for the psychanalyse all the same : Henri Chapier's article in Combat of October 24, 1969 is a striking example.

With this double contradictory movement, the story of Renee was consequently received in the world of psychiatry with a certain faintness which one could describe as theorico-private clinic faintness and one of the effects was that the story of Renee has been unfairly forgotten to take again the terms of the psychiatrist Lyonnais Jacques Hochmann in technic of care in psychiatry of secteur.

Naturalmente y por el hecho de ocuparnos de un caso que hacía pareja con el caso de Renée, nos conmovió en sumo grado este malestar inicial, exteriormente con relación a Renée, e interiormente a causa del trabajo que teníamos que reslizar con Laura. No obstante, por el hecho de su referencia a la historia de Renée, pensábamos con ingenuidad, hay que reconocerlo, que la acogida del caso de Laura no podría más que beneficiarse del reconocimiento pasado reservado a Renée.

En relidad, fue lo contrario. Lo que esperábamos que iba a jugar en favor nuestro se reveló como un talón de Aquiles. En su época, a Renée se la ensalzó. Había incluso tenido la suerte de resucitar dieciocho años después gracias al talento de un cineasta seducido también por su historia, pero este privilegio, en resumidas cuentas, no hizo más que ayudar a su embalsamiento, a prorrogar la prescipción inicial y a impedir la deconstrucción-disolución del malestar teórico-clínico que su historia había suscitado desde un principio.

Naturally and by the fact of being involved in the adventure of resumption of Renee's case, we could be only more sensitized with this initial faintness, at the same time of the outside compared to Renee and the inside by the means of work to be carried out with Laura. By doing this, and because of its reference to the story of Renee, we thought a little naively, we must recognized, that the reception of Laura's case could only profit from the last recognition.

Actually and quite to the contrary, what we expected to see as an asset was rather revealed as an Achilles' heel. In her time, Renee had been praised. She had even had the chance to come back to life eighteen years later thanks to the talent of a scenario writer allured in her turn by her history, but this privilege had in the end of race only taken part in making her mor attractive, in renewing the initial preclusion and in preventing the deconstruction-dissolution of the theorico-private clinic faintness which she had started at the beginning.

Teniendo en cuenta la importancia de esta situación problemática, hemos empezado por tratar el enfoque teórico que será consecutivo al caso de Laura (tratamiento psicoterápico de una joven esquizofrénica, contrapunto teórico) y aunque no nos hallemos en situación de ir más lejos y a pesar de correr el riesgo de ser demasiado concisos, enunciamos algunos puntos interesantes en extremo a modo de conclusión a esta presentación.

Taking into account the importance of these problems, we made our introduction of it in the theoretical approach which will follow the case of Laura (psychotherapy Treatment of a young person schizophrene ; theoretical counterpoint) and although we are hardly in situation of going further, let us state however, with the risk to be too concise some surprisingly interesting elements as a conclusion with this presentation.

Durante cada una de las etapas de la psicoterapia, resultó que sólos los actos administrados a un año y medio de intervalo habían sido terapeúticos: el don del agua, la lectura en consulta, el regalo del perro. Pero, más alla de este resultado, es posible profundizar el análisis del malestar teórico-clínico del que ya hemos hablado. Y no es ni siquiera porque nos hayamos basado en la teoría del psicoanálisis que esta haya sido solicitada desde el punto de vista técnico.

During each stage of psychotherapy, it proved that only the acts managed at one year and half of interval had been therapeutic : the gift of water, reading during the meeting, the gift of the dog. But beyond this result, it becomes possible to look further into the analysis of the theorico-private clinic faintness about which we had spoken. And it is not even because we used the theory of the psychoanalysis that, on the technical level, it was resquested.

En otros términos, y en lo absoluto : quienquiera que, hallándose en una situación equivalente, hubiera administrado los mismos actos en períodos similares, hubiera llegado al mismo resultado. Nos hallamos pues frente a una distancia entre la eficacia de la cura y los medios de comprender lo que nos ha llevado administrar los actos. Y esta distancia nos conduce incluso a no poder confirmar el modelo teórico al que nos referimos para alcanzar esta eficacia.
Esta paradoja nos sumió al principo en un malestar personal y nos ha obligado a admitir la constatación de que los resultados obtenidos no nos remitían ni a nuestras sutiles interpretaciones, ni a una hábil poda de nuestras defensas, ni a nuestros profundos análisis contratransferenciales y finalmente a un conocimiento de la técnica. Pero fue necesario que este malestar instalado entre Laura y yo fuera superado y se repercutiera en el malestar teórico-clínico que lo cernía para que nos volviéramos capaces de localizar y hacer el análisis de este segundo nivel de malestar que había sido presentido pero que nos había sido aprehendido por la mayor parte de los que la cuidaron.

In other words, and in the absolute, whoever, in an equivalent situation would have managed the same acts at similar periods, with or even without true theoretical support, would have led to the same result. We are the witnesses of a gap between the efficiency of the care and the means of understanding what led us to administer the acts, and this variation, even leads not to confirm the ideal model which we referred to reach this efficiency.
This paradox initially plunged us in a personal malaise and has constrained us to admit the report which the results obtained returned neither to our subtle interpretations, neither to a skilful pruning of defenses, neither to our deep against-tranferential analyses and finally neither to a knowledge of the technique. But it will have been necessary that this malaise existing between Laura and myself should be overcome and resounds with the theorico-private clinic faintness which was beyond so that one becomes able to locate and make the analysis of this second level of malaise which had been felt but not apprehended by the majority of the medical staff.

Se trataba pues no sólo de un tratamiento paradójico (como existen técnicas de exhortaciones paradójicas en teoría sistémica o bien en los tratamientos practicados por Victor Frankl en Viena) sino también de una cura paradójica en la cual cada uno se hallaba dividido entre la evidencia de la eficacia del tratamiento y la privación de los medios de representación de sus mecanismos. Claro está, no nos hemos limitado a estas constataciones y nos dirigimos a todos aquellos que quieran ampliar sus conocimiento sobre el análisis del caso de Laura para proponerles que nos manden sus señas por e-mail, a fin de informarles en cuanto nos sea posible sobre sus resultados.

It was thus a question not only of one paradoxical care (as there are techniques of paradoxical injunctions in systemic theory or the way in which Victor Frankl in Vienna practised) but of a paradoxical cure where each one was torn between an obviousness the effectiveness of the care and a deprivation of the means of representing the mechanisms of them.
